Medical Insurance (Burgess & Niple Employee Benefit Plan)
|
Network Providers: Coverage provided by specified group of physicians. $500
deductible per individual, per year, individual maximum expense is $1,500 per year.
$20 co-payment per office visit. After deductible, pays 90% of other expenses.
Non-network Providers: Coverage provided by non-featured group of physicians.
$1,000 deductible per individual, per year, individual maximum expense is $3,000
per year. After deductible, pays 80% of other expenses.
Other Covered Provisions: $75 co-payment for emergency room treatment. The
co-payment for retail prescriptions is $10 generic/$30 brand/$45 non-preferred for
up to 30 day supply. Mail order prescription available for maintenance drugs with
a $20 generic/$60 brand/$90 non-preferred co-payment per prescription (90 day supply).
Routine Eye Exam every 12 months for a $20 co-payment for Network Providers only
and Vision Eye Wear dollar reimbursement of $150 every 12 months when using a Network
or Non-Network Provider or a Cole Managed Vision Provider.

Dental Insurance (Burgess & Niple Employee Benefit Plan)
|
Maximum benefit is $1,500 per person, per year. Diagnostic and preventative services
paid at 100%. After a $25 deductible, pays 80% of most basic restorative and 50%
of most major restorative services. Orthodontics for children to age 19, with lifetime
max of $1,000.

Section 125
|
You may have deductions for premiums for our medical and/or dental plan, Health
Care Spending Account and Dependent Care Spending Account taken from your pay on
a pre-tax basis. There is a $1,500 limit on the Health Care Spending Account. For
Dependent Care, there is a $5,000 limit for a single person, $5,000 limit for a
married couple filing jointly, and $2,500 limit for a married person filing separate.

Paid Time Off (PTO)
|
PTO is used for any reason that requires you to be absent from work during normal
business hours and to receive wages. These absences include but are not limited
to holiday, vacation, sickness, injury, personal or bereavement.

Holidays
|
Holidays are included in your PTO account. Company recognized holidays are posted
on bulletin boards. Company offices will be closed on these days.

|
Short-Term Disability
|
Benefits begin on the 8th calendar day of disability and continue for 25 weeks at
70 percent of salary.
|
|
Long-Term Disability
|
Benefits begin after 26 weeks of disability. You receive 60% of salary up to a maximum
of $10,000 per month.

Life & Accidental Death & Dismemberment Insurance
|
$50,000 death benefit. Additional $50,000 in event of accidental death. Other levels
of benefit in event of dismemberment. Supplemental life is available for employee,
spouse and children.

Profit Sharing Plan with 401(k) Feature
|
You may contribute to the fund through a salary deferral 401(k) plan.

Professional Registration
|
The Company will pay for the original and annual fees for professional registration
in the State of an employee's employment if there is a business necessity and if
he/she will be actively using it.

Telework
|
Employees may work within the Telework program as determined by Company guidelines
with approval of supervisor.

Flextime
|
Employees may work a flextime schedule within Company guidelines with approval of
supervisor.

Parking
|
Free off-street parking is provided in all office locations.

Educational Assistance Program
|
Tuition reimbursement and lab fees for approved courses. There is a 2-course per
term and $4,000 annual limit per individual employee.

Seminars and Conventions
|
If a seminar is determined by you and your supervisor to be beneficial, then a request
form can be completed. The form is submitted through the proper channel for final
approval.

Society Dues
|
Annual dues for membership in one society or association will be paid by the Company.
Society or association must be on our approved list.
|
|
Recruiting Bonus
|
A recruiting bonus will be paid to an employee if someone they refer is hired to
fill a full time position.

Credit Union
|
Credit Unions are voluntary associations of employees incorporated to encourage
regular savings and to provide convenience in borrowing.
